Name Change – Newfoundland And Labrador

In Newfoundland & Labrador, the process to change your name is age-dependent. To legally change your own name, you must be 16 years of age or older. If you are under the age of 16, a parent or legal guardian must apply on your behalf. Regardless of age, all applicants must have been residents of Newfoundland & Labrador for a minimum of three months.

Adults must complete the Application for Change of Name of an Adult. In addition to the completed form, all previously issued original birth certificates must be submitted, alongside the required application fees.

To change the name of a child, a parent of legal guardian must complete the Application for Change of Name of a Child. Consent of the child’s other parent or legal guardian is required; where that is not possible, a Request for Waiver must be filled out on the application. If the child is between 12 and 15 years of age, they must also provide their consent. In addition to the completed form, all previously issued original birth certificates must be submitted, alongside the required fees.
